Latch terminal delivery failures before advancing the queue
When verification reaches this failure branch while another delivery is already queued, removing the failed ID leaves no pending verification or recovery, so the next pending_injection_interval tick admits the successor. The broker's BlockedOnSend state is only observational—delivery handoff does not consult it—so later instructions continue executing after an earlier instruction was dead-lettered; a late retry of the same ID can also be accepted again. Preserve a terminal failure latch and stop injection until explicit recovery or restart.

Fixed in #1954 (ca627bf7d). When a delivery whose body was already written fails terminally, the worker now latches it before dropping the pending id. The id goes into a PTY-lifetime failed-delivery fence, so a replay is answered with delivery_failed and never pasted again. A FailedComposerLatch also blocks next_injection_index until the composer no longer holds that draft (it is idle, or the failed tail is neither in the composer nor at the cursor). Tests: failed_written_delivery_latches_injection_and_fences_replay, failed_draft_latch_holds_until_the_composer_releases_it; each fails when its part of the fix is reverted. I'll resolve this thread once #1954 lands on trunk.

Update, now at a3cbca912 after review on #1954: the latch is stricter. It releases only when the composer is provably idle, or via an explicit operator-wide flush_injections; a tail missing from the viewport no longer counts as release. The replay fence is now unbounded for the PTY lifetime.

Move the new fix into the pending release section
This commit lands after the dated 13.2.0 release but inserts its new user-visible behavior into the older 13.1.1 section, leaving [Unreleased] empty and making the historical release record claim functionality that was not present there. Move the entry under a new [Unreleased - Patch] heading rather than rewriting an already published version.

Ignore empty PTY writes before transferring ownership
If a client sends a valid write_pty frame with data: "" while delivery verification or recovery is pending, this unconditional generation increment treats the zero-byte write as human ownership: pending verification is failed immediately, or an in-flight recovery fails when its generation is checked, even though no input reached the composer. Reject or no-op empty data before changing human_input_generation so harmless empty input frames cannot dead-letter a delivery.

This is a must-fix item from the #1939 review. The fix is in #1954 (draft, base trunk), which carries M6 (ignore empty write_pty; fail deliveries only on an admitted human write) with a regression test. Leaving this thread open until #1954 merges into trunk; it will then be resolved with the merge SHA.

Fixed in #1954 (ca627bf7d). write_pty now submits first, and only a non-empty write the drainer admitted bumps human_input_generation, cancels queued recovery, and fails pending verifications (human_write_takes_ownership). An empty frame or a full/closed queue leaves the relay delivery intact. Test: only_an_admitted_nonempty_human_write_takes_ownership (red when reverted). I'll resolve this thread once #1954 lands on trunk.

🔴 Failed deliveries can be injected again
When a parked delivery exhausts recovery, pending_worker_delivery_ids forgets it although its body remains in the composer. A fleet replay can then paste the same body again, creating duplicate or mixed turns.
Learn more
A terminal verification failure does not erase text from the child process's editor. The worker removes the pending identity here, so deliver_relay accepts a later replay as a fresh injection. Fleet deliveries retain their unacknowledged record after a worker failure and can be replayed; a manually redelivered dead letter also targets the same live composer with a new delivery ID. The resulting paste can append to the old draft or create a second turn.
Example: A Codex composer still contains delivery A after three submit attempts. A fleet replay of A reaches the same worker; its ID is no longer in the pending set, so the worker pastes A again over the parked draft.
Recommended fix: Preserve a terminally failed, already-written delivery fence for the lifetime of the PTY, and prevent fresh injections into a composer containing unresolved failed text. Define explicit operator recovery for redelivery instead of removing the dedupe fence immediately.

🔴 Composer repaint confirms unsent messages
When current_composer misses a parked body, any_output accepts a later repaint for harnesses without explicit activity patterns. The worker acknowledges an unsubmitted message and starts the next delivery.
Learn more
The generic ActivityDetector returns any_output for any nonempty post-echo output, including ordinary UI redraws. current_composer only examines text through the cursor, so the message tail can be present to the right of the cursor but absent from parked; it can also be unrecognized in a changed TUI layout. A redraw then reaches this acceptance branch without proof that the editor submitted the turn.
Example: Muse displays a pasted message, places the cursor before its last 96 compact bytes, and repaints the editor. The message stays in its composer, but parked is false and any_output releases the delivery ACK.
Recommended fix: Do not use generic PTY output as harness acceptance evidence. Require a demonstrated empty composer after echo or a harness-specific turn-start marker; leave unsupported layouts inconclusive.

Fixed in #1954 (ca627bf7d). The generic any_output path now rejects any draft whose compact tail ends at, starts at, or straddles the cursor (tail_touches_cursor), so a repaint of an unsent body stays Inconclusive. When a wrapped echo matches only by its compact tail, observe now maps the match end back to raw offsets and keeps the same-read suffix, so a turn marker in that chunk is retained. Tests: generic_repaint_cannot_confirm_a_draft_straddling_the_cursor, wrapped_echo_preserves_same_chunk_acceptance_activity, compact_match_end_maps_back_to_raw_offsets (red when reverted). 🟡 Failed delivery leaves worker permanently blocked
After a terminal delivery_failed, BlockedOnSend persists despite removing the failed delivery. Later output and idle events preserve that state, so a working agent stays marked stuck until another delivery succeeds.
Learn more
The failure path has already removed the matching pending delivery and moves it to the dead-letter store. The new state assignment sets BlockedOnSend, while worker_stream and agent_idle now deliberately leave that state untouched. A live worker with no further deliveries can therefore never clear the blocked status through normal output or idleness.
Example: A single delivery fails because its composer could not be verified. Its pending count becomes zero; the agent later produces output and becomes idle, but listings still show blocked_on_send.
Recommended fix: Track blocked-on-send against a currently unresolved delivery, or clear the state on terminal failure when no other delivery is awaiting acceptance.

🔴 Wrapped echo drops same-chunk turn activity
When only a wrapped message tail matches, observe cannot locate the full echo and discards activity in that chunk. A turn-start marker arriving with that echo is lost, leaving an accepted delivery unconfirmed.
Learn more
A native TUI can reflow a message by inserting line breaks. pending_verification_echo_seen then recognizes the compact tail, not the exact formatted string. This branch clears activity_buffer, and rfind on the exact string returns None, discarding any subsequent busy marker in that same read. If no later marker appears, the verification times out as inconclusive even though the harness started processing the delivery.
Example: Codex outputs a wrapped version of Relay message ... WRAPPED_CODEX_ACK immediately followed by Working (1s • esc to interrupt) in one read. Tail matching sets echo_seen, but the busy marker is not retained; the turn can be dead-lettered after five seconds.
Recommended fix: Find the end of the matched compact tail in the raw/normalized output and seed only the bytes after it; add a wrapped-echo-plus-activity-in-one-read test.

🔴 Delayed recovery Enter outlives human takeover
If human input arrives during Codex's delayed recovery write, human_input_generation marks it failed only after the write finishes. The queued CR still reaches the PTY and can submit the human's draft.
Learn more
Codex recovery enqueues a compound write that sends End, waits one second, then sends CR. Human write_pty may be queued during that pause. The generation check in the recovery ack handler runs only after the drainer completes the compound write, so it cannot cancel its CR. The same delayed write is shared by wrap-mode recovery.
Example: Recovery sends End at time 0; a human types a draft at 0.5 seconds. At one second the queued CR still arrives and can submit that draft, although the worker subsequently reports cancellation.
Recommended fix: Make the delayed submit cancellable at the PTY drainer before CR is written, or stop automatic recovery and await human input behind a fence that can cancel the follow-up gesture.

Fixed in #1954 (ca627bf7d). Recovery writes are now cancellable in the PTY drainer, which checks a shared flag before the primary key and again after the follow-up delay. A cancelled write acks Interrupted and the drainer keeps running. Admitted human input (write_pty in the worker, stdin in wrap mode) sets every queued recovery's flag, so Codex's delayed CR, or a queued CR/LF, never reaches the child. After a takeover, the recovery ack assesses real acceptance: a turn that already started is confirmed, not dead-lettered. Test: cancelled_followup_is_withheld_and_drainer_keeps_running (red when reverted). I'll resolve this thread once #1954 lands on trunk.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win
Do not leave the worker in BlockedOnSend after a failed delivery that has no pending work left.
A matched delivery_failed now sets handle.state = AgentWorkState::BlockedOnSend and publishes stuck. This change also removed the paths that used to clear that state:
worker_stream(Lines 1443-1445) keepsBlockedOnSend.delivery_active(Lines 1007-1009) keepsBlockedOnSend.agent_idle(Lines 1694-1730) skips bothIdleand theidletransition whileBlockedOnSendis set.
After these changes, only a new delivery_queued, delivery_injected, delivery_ack or delivery_verified event clears the state.
Here is a realistic trigger. A human types into the PTY while a delivery is being verified. The worker emits delivery_failed with "human PTY input took ownership…". The delivery is removed, so pending_delivery_count can be 0. The broker still emits AgentBlockedOnSend { pending_delivery_count: 0 } and publishes stuck. The healthy agent then stays blocked_on_send in listings, and its idle transitions are suppressed until another message arrives. The same thing happens after an "inconclusive" failure.
Fix: mark the worker blocked only while this worker still has pending deliveries. Otherwise, return it to Working.

Fixed in #1954 (ca627bf7d). After a matched delivery_failed, the worker becomes BlockedOnSend (with agent_blocked_on_send and stuck) only while another delivery for it is still pending. Otherwise it returns to Working, so later output and idle transitions apply. Tests: terminal_failure_without_remaining_work_does_not_block_the_worker, terminal_failure_with_remaining_work_stays_blocked_on_send. I'll resolve this thread once #1954 lands on trunk.

Failed delivery leaves worker stuck
Medium Severity
A terminal delivery_failed now sets BlockedOnSend after the pending delivery is already cleared. agent_idle, worker_stream, and delivery_active refuse to leave that state, so a worker can stay stuck with zero pending deliveries until a later inject. Human ownership cancellation takes the same path, so ordinary attach typing can freeze listings after a cancelled send.

Human input falsifies in-flight recovery
High Severity
Human write_pty bumps human_input_generation and fails the delivery, but an already-queued submit recovery still writes End/CR/LF. The recovery ack then sees a generation mismatch and reports delivery_failed without calling assess_harness_acceptance, so a turn that already started is dead-lettered and can be retried as a duplicate.

Failed parked body unblocks next inject
High Severity
After a parked delivery exhausts submit-key recovery, the worker emits delivery_failed and drops it from pending_verifications. next_injection_index then treats the composer as free, so the next queued message is typed into a composer that still contains the failed body and can submit both as one turn.

Valid. OpenCode has no explicit patterns, so it falls into the generic any_output tail of assess_harness_acceptance. That is the same defect as the M2 thread at delivery_verification.rs:419, so it is fixed there rather than with a separate OpenCode flag: #1954 only lets the generic path accept once the body's tail no longer touches the cursor (tail_touches_cursor), so a typed-but-unsubmitted OpenCode body stays inconclusive. Will resolve when #1954 merges into trunk.
There was a problem hiding this comment.
Fixed in #1954 (ca627bf7d) as part of M2. The guard sits in assess_harness_acceptance's generic any_output branch, which OpenCode reaches: post-echo output can no longer confirm a body whose tail is at or straddling the cursor. I did not add the separate OpenCode-only flag. The broker has no OpenCode composer-idle model, so requiring composer clearing would dead-letter every OpenCode PTY delivery. Test: opencode_output_cannot_confirm_a_draft_at_the_cursor (red when reverted). I'll resolve this thread once #1954 lands on trunk.
There was a problem hiding this comment.
Correction to my earlier reply: tail_touches_cursor no longer exists in the final code. The guard on the generic any-output path is now !tail_near_cursor(..) && !body_text_right_of_cursor(..), from #1954 (c749f74e/0202e9f8), and opencode_output_cannot_confirm_a_draft_at_the_cursor covers it. All of this lands via #1959.

Valid. current_composer anchors on the last prompt-looking row (rposition(is_prompt)), so a > b row inside a multiline body cuts the composer short, the tail check misses, and the parked draft reads as Inconclusive. Regression tests are on #1955: one with a > row inside a parked draft, plus a control where the submitted body sits above a fresh prompt. The fix is in progress on that branch; I will reply with its SHA.

Fixed in #1955 (a80f9202, base trunk). composer_holds_tail keeps the latest-prompt containment check and also accepts an earlier prompt anchor when that composer ends with the body tail at the cursor, so a > b row inside a draft no longer hides it. Regression test: prompt_like_body_rows_do_not_hide_a_parked_draft (red on trunk, green now), plus a control showing that a submitted body above a fresh prompt is not treated as parked. - `--state-dir` on `node agent` commands, `node status`, and `node down` also accepts a fleet node directory whose broker state lives in `state/`.
- An explicit `--state-dir` on `node agent` commands is no longer overridden by `RELAY_BROKER_URL` / `RELAY_BROKER_API_KEY`.

P3: This entry was added under [13.1.1] - 2026-10-04, an already-released section (13.1.2 onward, including 13.2.0, are listed above it with dates), yet its own last sentence says the fix ships in the 13.2.0 rollout and ## [Unreleased] at the top is empty. Per the AGENTS.md changelog rules, new pending user-visible changes go under [Unreleased], promoting the heading to [Unreleased - Patch]/- Minor per SemVer impact. As placed, the 13.1.1 release notes claim behavior that never shipped in 13.1.1, and the release where it actually ships carries no note. Move the bullet to [Unreleased] (or the not-yet-cut release section).

Rust tests/clippy/fmt/cross-compile, Node test/coverage/lint, build-bun.sh tests and publish Build & Version use vars.CI_LINUX_RUNNER with an ubuntu-latest fallback; matrix jobs switch only their ubuntu-latest entry, so check names are unchanged. macOS/Windows jobs are untouched. The PR-proof Landlock/devpts/no_new_privs tests self-skip on kernels that cannot host the sandbox (including StarSling's 6.1 kernel), so a new sandbox-proof job pinned to ubuntu-latest runs that file and fails if any of its tests is skipped. StarSling Linux runners preinstall rustc 1.92.0; GitHub's ubuntu-24.04 image ships 1.99.0. The broker uses atomic try_update (#1907), which 1.92 rejects as unstable (E0658), so test, Coverage (upload) and Test build-bun.sh failed on trunk -> main #1939 after #1946 routed them to vars.CI_LINUX_RUNNER. Rust CI jobs already install stable via dtolnay/rust-toolchain and passed on the same runners. Co-Authored-By: Claude Opus 5.5 (1M context) <noreply@anthropic.com>

Changes core PTY message delivery, fleet ack gating, and failure/recovery semantics—incorrect acceptance detection could drop, duplicate, or block agent turns across production brokers.
Overview
PTY broker delivery no longer treats terminal echo (or a timeout) as success. Messages stay pending until the harness proves a turn started—activity markers, a cleared idle composer, or (for
cat) process echo—and failed or parked delivery blocks further sends instead of silently queueing more input. When the body is still visible in the live composer, the worker may run up to three submit-only recovery attempts (End/CR/LF style keys viasubmit_injection_recovery, without replaying the body); humanwrite_ptyinput cancels automatic verification and fails the in-flight delivery. Broker/runtime wiring addsdelivery_unconfirmed/delivery_resubmittedobservability,harness_acceptance+ evidence ondelivery_verified, and keeps agentsBlockedOnSenduntil acceptance clears. Supporting changes include richer composer/snapshot detection (Codex, Claude, Gemini, Devin), expanded activity detectors, and extensive tests (plus ignored live-Codex probes).CI can target optional StarSling Linux runners via
vars.CI_LINUX_RUNNER, with explicit stable Rust on those images and a dedicatedsandbox-proofjob on GitHububuntu-latestso Landlock PR-proof tests cannot be skipped on incompatible runners. CHANGELOG and Devin harness docs document the new acceptance/recovery behavior; AGENTS.md merge-train snippet formatting only.Reviewed by Cursor Bugbot for commit da7d54d.
