Add Layer-based service provisioning, request-scoped Effect providers, Effect-native generator middleware, contiguous runtime-boundary batching, and FiberRef continuity across split Effect groups. Add Knip to the repository checks and clean up existing unused exports, stale test fixtures, and the unused ultracite dev dependency so the new check starts from a clean baseline.
