fix(policy): enforce HTTP admission for inference fallbacks - #1520
Enforces HTTP policy for inference fallbacks without a model, and preserves credential stripping, delegation, and assertions during ordered header mutations. Also adds some regression tests for Pingora and Hyper.
Summary by CodeRabbit
Security
Accepted requests without a model now receive HTTP authorization, delegation, and request-header checks when model requirements are disabled.
Discovery requests are subject to HTTP policy. Credential stripping, delegated authorization, and asserted headers are applied consistently through ordered request processing.
Documentation
Clarified the effects of disabling model requirements and where inference-specific rules apply.
The policy filter now applies HTTP admission to bodyless inference requests and accepted unclassified requests without a usable model. Shared header helpers coordinate policy mutations with ordered pre-read mutations. Unit and integration tests cover admission outcomes and header handling.
Shared helpers enumerate and purge pending header mutations, then set or remove headers. Assertions and delegated credentials use the helpers. Unit tests cover mutation ordering and header precedence.

Bodyless inference requests and accepted unclassified requests without a usable model use HTTP admission. Tests cover GET and POST cases, authorization outcomes, and warning expectations.

Integration tests cover fallback admission, discovery requests, delegation, and ordered header mutations. The example configuration and changelog describe the fallback behavior.
praxis-proxy/praxis#1516: Adds HTTP authorization, delegation, and assertions for bodyless LLM requests, which connect to this PR’s inference fallback behavior.

Explanation
Docstring coverage is 61.90% which is insufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff with cyclomatic complexity of at least 2. Documentation-only edits and deletions within surviving functions are included; unchanged diff context and fully deleted functions are excluded. Analyzed 21 functions across 9 files.

LGTM. Found no unintended policy/ppe regressions.
There is an intentional compatibility change: with llm.require_model: false, model-less requests now run HTTP policy. Inference-only global rules that read llm.request can therefore reject these requests; those rules should be placed under llm: routes or global.defaults.llm.
