fix(server): consumed Claude background wakes no longer block the thread - #26553
Unproven: the original macOS timing race with a live Claude CLI, and full orchestrator delivery of a queued delegated completion. Verification uses controlled SDK frames in the real Claude adapter runtime on physical corex Linux.
A Claude background wake can buffer its opening frames, then finish while an earlier queued user run is active. The later notification run replayed those stale frames as evidence that another native turn had started. It then waited indefinitely for a result Claude had already delivered, blocking following runs.
Mark buffered root frames when a later native result closes their turn. Replaying those frames now reaches the existing wake settlement logic. Buffered output remains available and no duplicate prompt is sent. This focused server bug fix changes only the adapter and its existing regression test; no wire contracts or client code change.
Verification at 2f1562cad7faca5bf203b199852f3a82c24f610e on corex Linux:
The extended existing regression fails on base 5211acfe95a0c6d82a60e2524b5e46b47fe67a3e. After eight simulated hours, the stale wake still has no terminal event. The original fully buffered result case passes.
Both regression rows pass with the fix, including a following user turn. All 245 Claude adapter tests pass.
vp run check:changed passes formatting, lint, both knip passes, GraphQL generation, affected-package typechecks, and the adapter tests.
Independent GPT-6.1-Sol review found no issues at this commit.
Controlled runtime transcript records the background completion, intervening user result, stale continuation settlement, and following user completion. It offers exactly three user prompts, with no prompt for the stale continuation.
Controlled adapter runtime transcript, with injected SDK frames and no test-clock advance:
initial-completed ["completed"]
user-consumed-result ["completed","completed"]
continuation-completed-without-clock-advance ["completed","completed","completed"]
next-user-completed {"terminals":["completed","completed","completed","completed"],"prompts":["Ultrathink:\nRun the build in the background.","Ultrathink:\nHow is the build going?","Ultrathink:\nContinue with the next task."]}
The regression on the base reports expected length 3 but got 2. The fixed regression reports 2 passed; the full adapter file reports 245 passed.
Implemented with Claude Opus 5.5 in Claude Code; investigated, tested, and reviewed with GPT-6.1-Sol in Codex through T3 Code.
…their wake
An idle Claude wake can buffer its init or assistant frames, then finish
with a result that lands in a queued user turn. The continuation later
replayed those stale frames, treated them as a live wake start, and
skipped both no-start safeguards, so it stayed running and blocked the
thread queue.
A live root result now marks buffered root frames as ended. Replaying an
ended frame no longer counts as a wake start, so the existing safeguards
settle the continuation while buffered output still replays.

Macroscope's review found this PR approvable — Consumed Claude background frames no longer make a replayed continuation appear newly active, allowing existing completion handling to release the thread. The change is confined to in-memory wake bookkeeping and preserves message routing.

The Claude adapter now tracks buffered root-turn frames whose wake turn ended after a live result. Continuation wake-start detection ignores those frames. Tests cover buffered wake results and wake-turn initialization frames.
The adapter marks buffered root-turn frames as ended when a live query emits a result. Continuation wake-start detection ignores those frames. Tests cover both buffered message types and verify continuation behavior.
Buffered assistant output lacks a regression check for consumed wakes; the change is otherwise mergeable with that follow-up.

The whole-PR diff against merge base 5211acf touches two files. ClaudeAdapterV2.ts (+10/-1) contains the stale-frame fix. ClaudeAdapterV2.test.ts (+108/-81) extends the existing buffered-wake regression. Both files are in the Claude adapter under apps/server, the area named by #26537. The test changes support the fix. They add the buffered wake-turn init row, the 8-hour clock advance, the pending-background-work check, and the following user turn. No wire contract, client, or unrelated file changes appear in the diff. This matches the PR description.

Explanation
[#26537] Stale-continuation requirement: The diff adds endedWakeTurnFrames in apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts. When a live result arrives, buffered root-turn frames are marked as ended. A continuation with no prompt UUID no longer sets nativeWakeStarted from those frames. The extended regression in ClaudeAdapterV2.test.ts covers a buffered task-notification result row and a buffered wake-turn init row. It checks that the continuation reaches a terminal event after TestClock.adjust("8 hours"), that the buffered result is emitted only when the continuation runs, and that no duplicate prompt is sent. This supports the adapter-level part of the expected behavior. Queued runs: The regression sends one following user turn and checks that it completes. This covers the adapter only. Delegated-task completion: No code or test in this PR covers orchestrator delivery of a queued delegated-task completion to its parent. The PR description states this is unproven. The original macOS timing race is also not reproduced. Verification uses injected SDK frames. Test results (245 adapter tests, check:changed) are author-reported. This review did not run them.
Resolution
Provide evidence that a queued delegated-task completion reaches its parent after the fix. Acceptable evidence is an orchestrator-level test or runtime trace that delivers a queued delegated completion behind a stale continuation. Provide evidence that the stale buffered-frame path is the mechanism of the reported timing race in #26537. Alternatively, state that the adapter-level fix is the intended scope of #26537.

Cover buffered assistant output in the consumed-wake regression.
The regression buffers only wakeResult or wakeTurnInit. It does not buffer an assistant frame. The existing assistant test uses a different ordering. Therefore, this regression can pass if buffered assistant text is lost during consumed-wake replay.

Thank you @shivamhwp for tracking this race down. #26554 fixes the same issue (#26537) in the same adapter, covers both frame orderings with a regression test that fails on main, and has an uploaded runtime transcript and two reviews, so we're keeping that one and closing this as a duplicate.
