This introduces a styling regression, because there are a bunch of rules in the stylesheet that were never used previously that are now enabled. I'll do a fixup for this in the next change.
This removes a bug where entries would overlap their label. It deletes a defunct .indexIndex rule that did nothing even when enabled. It removes the `-moz-` prefix on the two column code, which is long unsupported, even in Firefox.

This will re-enable styling of the index.
Unfortunately, this also re-enables style rules that were badly broken. I've had to fix them at the same time.
