  1. Procurement of Computers What is needed for computer procurement

  3. The instructions for travel are as follows:

    1. Download the permission form from https://www.fuw.edu.pl/dokumenty-i-formularze.html, fill it in and submit it
    2. Google the preferred flight
    3. Contact travel agency and ensure prices are similar
    4. Fill in the ticket requisition form at https://www.fuw.edu.pl/dokumenty-i-formularze.html using the WhyNotTravel booking reference
    5. Get the ticket requisition form signed. Once signed, order the ticket.

  4. For domestic travel you may need the NIP and Address. It is: Uniwersytet Warszawski Krakowskie Przedmieście 26/28 00-927 Warszawa, Poland NIP PL 535-001-12-66 The form has been updated, it is now NationalDelegationForm.doc and needs to be filled in electronically.
