Adds the FormCell applier for pp-form-cell. Reuses the shared locator/placement resolver and adds FormScaffoldFragment, which picks the dialogcell.xml fragment when the located form turns out to be a dialog (auto-detected from the folder when form-type isn't given).
Stacked on #88 (the shared parameter mapping lives there), merge after it. Same parity result as row: byte-identical output, plus the form-id-only variant the old locator crashed on.
