k-means gives an empty cluster a zero center, and the cosine coarse distance to it was NaN. select_k never picks NaN columns, so when fewer than n_probes lists have a finite distance the remaining probes are padded with list 0, which is then scanned several times and returns duplicated neighbors. Define the cosine distance to a zero-norm center as 1, as IVF-SQ already does, and add a regression test.

Notes for reviewers:
- The original flake is already fixed on
main, and this PR doesn't change that.NEIGHBORS_ANN_IVF_FLAT_TESTAnnIVFFlatTestF_{int8,uint8}.AnnIVFFlat/2(dim = 2,CosineExpanded) failed intestFilterwith recall 0 or duplicated indices 6 times between 2026-08-12 and 2026-09-19, mostly on12.2.2, amd64, rockylinux8, v100, earliest-driverand once on H100. There have been 0 hits since. A bisect showed that #2249 fixed it: on V100, 82 of 4800 runs failed at 0396eb4 (just before #2249), 0 of 4800 at 543fe19 (#2249), and 0 of 4800 on currentmain(bisect table). This is workstream N in #2783; the reproducer PR is #2815. - What failed: with dim = 2 int8 data there are only about 220 distinct directions for 1024 lists, so balanced k-means leaves most clusters empty and gives them zero centers. Before #2249, the V100 k-means
predictpath returned NaN for zero centers and sent every vector to list 0. IVF-Flat's coarse search then also produced NaN for those zero-norm centers (root-cause comment). - What this PR does: it is a defensive guard for that last step, which is still present on
main. If more thann_lists - n_probeslists have zero-norm centers, the probe list is padded with list 0, and search returns duplicated neighbors. The guard defines the coarse cosine distance to a zero-norm center as 1 (similarity 0), as IVF-SQ already does. The newAnnIVFFlatTest.CosineZeroNormCentersfails without the guard (Duplicated index) on all 4 jobs (3x V100, 1x H100) and passes 20 of 20 with it, and the fullNEIGHBORS_ANN_IVF_FLAT_TESTpasses (run 38107679187). - Question for reviewers: do you want this guard, or would you rather rely on #2249 and close this PR? With #2249, zero centers no longer collect all the data, so the remaining trigger needs more than
n_lists - n_probesempty lists, for example a dataset with fewer distinct directions thann_lists. I haven't seen that trigger in CI onmain. - Question for reviewers: the cosine ops that
fusedCosineNNuses (fused_cosine_nn.cuhL56-L82) compute1 - acc / (x_norm * y_norm)with no zero check (CUTLASS epilogue L14-L23, SIMT epilog L61-L79), so they may also give NaN for zero-norm vectors. Is a guard there worth a separate follow-up, or are those paths no longer reached with zero-norm inputs since #2249?

For cosine, the GEMM runs with alpha = -1 (L130-L162), so dist is -q.c, and this map turns it into -cos(q, c), which select_k minimizes. A list whose cluster ended up empty has a zero center, so its norm is 0 (center norms are computed in extend, ivf_flat_build.cuh L339-L355), and dist / denom becomes 0 / 0 = NaN. select_k never picks a NaN column. When fewer than n_probes columns are finite, the remaining probe slots get index 0, so list 0 is scanned several times.
Returning 0 here means similarity 0, i.e. cosine distance 1. A zero-norm center then ranks after every center with positive similarity, so it is only probed when there are not enough better lists. This matches IVF-SQ's coarse search (ivf_sq_search.cuh L460-L474) and the convention in the cuTile 1-NN kernel from #2552 (fused_1nn_kernel.py L132-L137).
The guard also changes the zero-query case: every list now gets 0, so the probes are a select_k tie-break. Before, every list was NaN, so all probe slots were list 0.

The test builds the bad case directly instead of relying on k-means to leave lists empty. It builds with add_data_on_build = false, zeroes centers 1..1000, and then calls extend. Center norms start unset (ivf_flat_index.cpp L43) and are computed on the first extend, so they reflect the zeroed centers. That leaves 24 lists with a non-zero center, fewer than n_probes = 40, so without the guard 16 probe slots are padded with list 0. With the guard, all 24 non-empty lists are probed, the search is exhaustive over the data, and it is compared with naive_knn at a 0.99 recall threshold.
Question for reviewers: editing idx.centers() before the first extend depends on norms being computed lazily. Is that acceptable, or would you prefer to create the empty lists through the data, for example int8 data with few distinct directions, as in the original failure? That would depend on k-means behavior and would be harder to make deterministic.

k-means gives an empty cluster a zero center. In IVF-Flat's cosine coarse search, the distance to such a list was
dot / (|q| * 0)= NaN.select_knever picks NaN columns, so when fewer thann_probeslists have a finite distance, the remaining probe slots are padded with list 0. List 0 is then scanned several times, and the search returns duplicated neighbors. This defines the cosine distance to a zero-norm center as 1 (similarity 0), as IVF-SQ already does, and adds a regression test (
AnnIVFFlatTest.CosineZeroNormCenters). Without the guard the test fails on V100 and H100 withDuplicated index; with it, the test passes (run). This is the search side of the flaky
AnnIVFFlatTestF_{int8,uint8}.AnnIVFFlat/2failures (recall 0 / duplicated indices) tracked in #2783. Their k-means side was fixed by #2249.
