Bump version to 0.5.0 and document the layered rewrite in the changelog.
Prometheus is no longer the default backend, so drop it from the one line description shown on PyPI.

Prepares the 0.5.0 release.
Breaking change for importers: the flat kempner_pulse module is gone, so imports move to the kempnerpulse package. The command line is unchanged apart from the dcgm default backend and backend-aware poll.
Verified: 156 tests pass, twine check passes on both artifacts, and the wheel installs into a clean venv where kp reports 0.5.0 and the replay backend renders.
