Cut the 0.4.0 changelog section (mp2ts-loop, POC/QP in mp2ts-nallister, build-info versioning, Go 1.24). Add the missing entry for the mp2ts-timeshift tool (92b3ddc). Point the changelog version links at GitHub compare URLs; the releases/tag/vX...vY form did not resolve. README: -version now comes from Go's build info, not the Makefile.
